ACM Care (Autonomous Case Management, Inc.) is a non-profit agency that was established in 1999 by co-founder and Registered Nurse Julie Hess. The agency was created after Hess witnessed elderly, disabled, and seriously ill patients in hospitals who didn't have the support of family. These patients were often told by medical professionals that they had no choice but to be admitted to a nursing home. Hess knew that there were other options available and decided to help.

Experienced Medical Staff
Our medical professional staff consisting of registered and licensed practical nurses have over 50 years of combined experience.They are highly qualified to oversee the healthcare ofyou or your loved ones. Our managers are certified in Case Management through the American Nursing Association and the Case Management Society of America. As members of the National Association of Professional Geriatric Care Managers, we have the knowledge that families need. Our nurses have over 15 years of experience and hold active licenses in Illinois or Missouri.
Experienced Geriatric Care Managers
Our Geriatric Care Managers have over 60 years of combined experience and hold bachelor's or master's degrees in social services or related fields. They are also required to attend over 24 hours of continuing education in care management each year. In addition, our Geriatric Care Managers regularly participate in multidisciplinary teams outside our organization, which helps to expand our network of professional contacts.
